The Role
The post holder will be a Registered Nurse who will support the community nursing service in the delivery of the highest quality of health care and will be responsible and accountable for providing skilled nursing interventions as appropriate. The post holder will ensure a person centred approach to care delivery which is safe and effective to ensure and maintain quality strategy. The post holder will also be expected to provide an accessible, appropriate, high quality and effective care service to all age groups in the community, by working in partnership with patients, care providers and other agencies.
In This Key Role, You Will
The post holder is responsible and accountable for assessing, planning and implementation of programmes of care. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.
Be fully compliant with the NMC Code of Professional Conduct and other relevant guidance documents, which support and maintain standards of professional practice for nurses and midwives.
Demonstrate a clear understanding of organisational goals
Demonstrate the ability to function competently both as an individual and team member, and understand and acknowledge the role and function of others in the development of effective partnership working
What You'll Bring
Registered Nurse with valid NMC Registration
The post holder will be required to demonstrate excellent team working skills with the ability to work using own initiative
Effective listening, communication and interpersonal skills
Ability to work with people and as part of a multidisciplinary team
Excellent time management skills
Working knowledge of basic information technology
A driving licence is required; this must be a full UK/EU/EEA licence.
